Emailed documents will not be processed. Complete an enrollment form for each instructional term and postsecondary institution (psi) the student attends. Once sections 1 and 2 are completely filled out, the next step is to work with the enrolling postsecondary institution to complete section 3.

If needed for eligibility, test scores; Please complete this nosr form, fill out sections 1 and 2, including a signature from your school counselor or administrator and send it in to the pseo email. We will contact you before the start of fall semester to request a form signed by your new school.
